Descripción y detalle de las actividadesCoordinate with other project staff and partners to provide housing placement and supportive services to facilitate the client's successful transition to permanent housing and coordinate services to foster housing stability during and beyond interim housing.Assist clients with addressing their immediate needs (e.G., access to emergency shelter, food, water, hygiene products, clothing, etc.)Connect clients to the homeless system of care (Coordinated Entry System), entry into HMIS, entry into CRM, and referrals to housing resources.Monitor and follow up with clients, both County and non-county providers and partners, to confirm the timely completion of linkages, services, and/or housing.Determine if clients receive Medi-Cal and have a primary care medical/and or mental health provider.Document the client's health and mental health changes. If health declines, engage with the client's health and/or mental health provider.Monitor and follow up with clients once they have achieved their care plan recommendations within the outlined timeframe to advance them to the next step.Participate in weekly case conference meetings with the Program Supervisor.Assist clients in obtaining additional needed services, including, but not limited to, mainstream benefits, housing linkages, substance use and mental health services, employment and vocational services, education, legal services, credit counseling, money management, tenancy education, landlord relations, life skills, and family reunification.Complete a CES survey packet tool for all participants who have not previously been administered the packet.Attend weekly case conferencing meetings.S/he will assist clients with identifying the most appropriate housing intervention.Coordinate with other FMWC staff and collaborative partners to provide ancillary services to achieve housing stability; follow up with clients weekly, at a minimum; maintain interagency consultation, coordination, and referrals.Experiencia y requisitosWorking knowledge of Microsoft Word, Excel, and other database programs. Knowledge of resource development, case management, and documentation. Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds. Ability to communicate effectively, both written and orally.
