Community Paramedics visit patients in their homes but also attend mandatory meetings at Cleveland Clinic Business Operations Center.
Each Community Paramedic has a primary territory assigned.
Based on what referrals come in for the day, schedulers make every attempt to keep each Paramedic in their primary territory, but Paramedics may be pulled into other areas as needed.
Primary Assignments include Southeast, South of Beachwood and North of Akron.Job DetailsA caregiver in this part time role, works various days, Monday – Friday from 8:30am – 5:00pm.A caregiver who excels in this role will:Provide home-based care that supports optimal health and wellness for the patient, within the scope of practice of a paramedic (EMT-P), under the direction/supervision of a Licensed Independent Provider (LIP) and appropriate health team members.Perform assessments and report findings of both in-depth and focused healthcare assessments, screenings and examinations to the appropriate team member(s) (this may be done using tele-presence, telephone and/or written documents).
These assessments take place in a residential environment requiring the caregiver to operate a motor vehicle and practice in a variety of community settings and locations.Provide supportive care and education with limited interventions.Provide basic cardiac and advanced life support as outlined in existing Cleveland Clinic EMS protocols in coordination with local Emergency Medical Service providers.Examine, screen, treat and coordinate health services for patients (may include point of care laboratory testing based on paramedic's education, training and competency).Conduct/assist with post-hospital care such as home safety/living condition evaluations, medication adherence/education, dressing changes, checking of vital signs and reinforcement of post-discharge/post-office visit education
