.**About this Position**We want to build a core team that will drive key transformation in US Healthcare management services.For this position, we are looking for an experienced Nurse or a Certified Case Manager with at least 5 years of clinical experience with direct patient care. An active License is NOT required. The RPM/ CCM Care Program - called Rev+ at Neolytix, is a program rolled out at multiple medical practices nationwide. The Rev+ Care Coordinator position is a vital member of a cross-functional care team driven by their mutual devotion to serving others. Client's Practice Management leans on this team to supplement the care they cannot provide themselves. Our portfolio of services caters to micro verticals and is built on the expertise we have developed in enabling these practices.Work with a company where your work can make a real impact!- We are a boutique company respected and loved by our clients providing no-nonsense advice on key issues that impact them.- 4.7 ? on Google and 4.2 ? on Glassdoor with 80% of approval rating!**Roles & Responsibility**- Onboard new Practices and operationalize a Care plan for their patients- Manage a team of case managers - Delegate & monitor RPM and CCM-related targets assigned to each case manager.- Coach and mentor case managers to improve productivity and care being provided to patients.- Act as the key point of contact between the Neolytix team and the Practice Provider- Identify patients for specific case management and/or chronic condition management activities using established screening criteria.- All other duties, as assigned.**Skills & Experience**- Licensure: Active license is NOT required.- Education: Bachelor of Science in Nursing (BSN) or Masters in Nursing.- Certification: Certification in Case Management (CCM) is preferred.- Experience: 3+ years of clinical experience with direct patient care required; 5+ years preferred.- Experience in case management/coordination of programs and services preferred.- Excellent verbal and written communication skills. Comfortable communicating with members and providers via phone regularly throughout the day.- Ability to communicate effectively with members and providers in implementing clinical services; Translate complex clinical concepts for non-clinical audiences.- Strong administrative/technical skills; Comfort working on a PCusing Microsoft Office (Outlook, Word, Excel, PowerPoint), IM/video conferencing (Teams & Zoom), and telephones efficiently
