Follow up timely as per timelines givenResponsible to get Discharge Confirmations from the referring agency according to set timelines givenExcellent communication with team members including the Business Development Liaison and/or referral source to update on patient status etc.Communicate timely and appropriately with team members and partners to ensure smooth delivery of serviceMonitor referrals received in RainTree Fax throughout the day and forward to appropriate Business Development Liaison/DirectorRun Weekly reports on overdue Plan of care on RainTree and document progress in spreadsheet
